Opening of a resort hospital in Macau signals a pivot from casinos to medical tourism

This week, Macau launched what is being called a resort hospital — iRad Hospital — offering premium-tier medical services. The project is designed to help the region, which surpassed Las Vegas in gaming revenue nearly two decades ago, build an economy no longer anchored to roulette and baccarat.

The new facility’s offerings include comprehensive health check-ups, advanced diagnostics using state-of-the-art scanning technology, and cosmetic and aesthetic procedures. The project’s operator expects that the medical proposition will encourage visitors to stay longer and spend more in the region.

Luxury medicine inside an entertainment complex

iRad Hospital is located within Studio City, a large-scale casino and entertainment complex styled after Hollywood, owned by a Hong Kong-based group. The placement of a clinic inside a casino resort is itself symbolic: it represents an attempt to rebrand Macau’s familiar image not by dismantling it, but by adding a new layer on top.

According to the private medical group behind the project, Macau receives approximately 40 million visitors annually. Converting even a fraction of that flow into patients at premium clinics could meaningfully shift the structure of tourist spending.

Why authorities are changing the revenue model

The Covid-19 pandemic exposed Macau’s core vulnerability. Casinos were forced to close, and revenue collapsed to near zero. Although the region’s economy has since recovered, the lesson proved painful: tax receipts from the gaming industry still account for the bulk of government revenues.

Officials are now advancing a policy of redirecting the economy toward healthcare, technology, and the events industry. The goal is not to shut down casinos, but to develop industries alongside them that are capable of generating comparable added value.

An additional source of pressure on land-based casinos has been the rapid growth of online gambling. Digital platforms are gradually drawing away audiences by offering what physical venues cannot — aggressive bonus programs.

Pressure in favor of diversification comes from the very top. During a visit in late 2024, Xi Jinping stressed the need for “economic diversification” and the development of “new industries with international competitiveness.” The gambling capital’s ostentatious luxury and its reputation as a playground for the wealthy appear to sit uneasily with the “common prosperity” agenda being advanced by the Communist Party on the mainland.

The political context is shifting as well. Unlike Hong Kong, where the pro-democracy movement openly challenged Beijing, Macau has traditionally maintained a more stable relationship with the central government. In recent years, however, observers have noted a tightening of control:

• The most recent elections to the Legislative Assembly recorded low turnout after some candidates were barred under new legislation.

• In early 2023, one of the region’s most prominent gaming magnates was sentenced to 18 years in prison on organized crime charges.

• A national security law targeting foreign interference and opposition to Beijing has entered into force.

Why medical tourism looks like a fitting alternative

Asia has long been an established hub for medical tourism. Affluent patients travel from around the world to South Korea for cosmetic surgery, to Singapore for high-technology treatments, and to India for more affordable procedures. Macau, with its well-developed hospitality infrastructure and enormous tourist flow, has the resources to position itself on that map.

The global medical tourism industry is valued in the tens of billions of dollars, and market forecasts point to significant growth over the coming decade. For a region that spent decades relying on a one-trick economy, this is a window of opportunity that Beijing clearly has no intention of letting pass.
